Privacy Notice for Patients
How we use your personal information?
This notice tells you why Malton & Easingwold Chiropractic clinics collect information about you and
how this information is used.
The Chiropractor who provides your care, maintains records about your health and any treatment or
care you have received here or previously. These records help provide you with the best possible care.
Our records are on paper only and your information is kept confidential and secure. Records that the
clinics hold about you may include the following information:
- Details about you, such as your address, contact details, previous medical history and previous investigations.
- Any contact that the Chiropractor has had with you, such as appointments, clinic visits, advice given over the phone, etc.
- Notes regarding your current and previous health.
- Details about your treatment and care.
- Relevant information from other healthcare professionals.
Information may be used within the clinic for clinical audit purposes to monitor the quality of the service
we provide. All of your information is held securely and is not shared with anyone not involved in your
treatment.
All paper records shall be held under the legal basis for 8 years after your last visit. After this point, all
data disposed of will be appropriately destroyed.
How do we maintain the confidentially of your records?
We are committed to protecting your privacy and will only use information collected lawfully in
accordance with:
- General Data Protection Rules 2018.
- Human Rights Act 1998.
- Common Law Duty of Confidentiality.
- General Chiropractic Council Code of Conduct.
The Chiropractor working at Malton & Easingwold Chiropractic clinics has a legal obligation to keep
information about you confidential.
Who do we share your information with?
The Chiropractor will only ever pass on information about you to others if there is a statutory obligation
to do so and you have given consent. This maybe to your GP, dentist, another healthcare professional or
a solicitor.
The Chiropractor will not disclose any information about you to any third party without your written
permission, or in the case of a child’s information, without the parental consent, unless there are
exceptional circumstances, where the law requires this information to be passed on in accordance with
the Caldicott principles.
At no time will your information be passed to organisations external to the company or for marketing or
sales purposes, or for any commercial use without your prior consent.
Access to your personal information
You have a right under the General Data Protection Rules 2018 to request access to view or obtain
copies of what information Malton & Easingwold Chiropractic clinics hold about you and to have it
amended should it be inaccurate. In order to request this, you need to do the following:
- Your request must be in writing to the clinic.
- There are no charges for copies of your file.
- We are required to respond to you within 40 days.
- You will need to provide photo identification to verify your identity.
Objections/Complaints
Should you have any concerns about how your information is managed at the clinic, please contact the
Chiropractor (Melanie Clark). If you are still unhappy, you can then complain to the Information
Commissioners Office via their website (www.ico.gov.uk)
Change of details
It is important that you tell the Chiropractor if any of your details such as your name or address have
changed, or if your details such as your date of birth are incorrect in order for us to correct it.
